House
Call Program
The goals of the House Call Program is to provide quality
cost effective services to the homebound persons who are unable
to visit the doctor’s office without a great deal of
difficulty due to limited mobility, cognitive impairment and
/or transportation constraints. Services include: physical
exams, regular scheduled visits, prescription refills and
referrals to appropriate services.
The
benefits of the House Call Program to patients, the hospital,
and the community can be considerable. Patients benefit because
at home care helps identify new problems and allows appropriate
referrals to be made in a timely manner. The program also
enhances patients’ quality of life and ability to live
independently longer, and limits or delays the utilization
of emergency department, hospitals, and nursing homes; it
also allows the patient to live and die with dignity.
Under the direction of Dr. Delbra Caradine, Geriatricians,
and APNs provide primary medical care at home while collaborating
with traditional home health agencies. This service if available
to individuals 65 and older or young adults who have diseases
of the elderly and live within a 30-mile radius of UAMS.
This is not an emergency service. Appointments or referrals
can be made by contacting 501-296-1000, Monday through Friday,
8:00 a.m. – 4:30 p.m. We accept self referrals as well
as referrals from any other home health agencies or healthcare
providers.
Medicare, Medicaid, private insurance or private pay can
be used to pay for the service.
